Access Your Health Records & File a Complaint
Texas:
Under
House Bill 4224 (89th Regular Session), Section 181.105 of the Health & Safety Code, effective
September 1, 2025, consumers have the right to:
- Request health care records by submitting a written Request for Release of Information (ROI) including your name, date of birth, contact information, description of the records, signature, and date.
- Contact the Texas Behavioral Health Executive Council (BHEC) for questions about licensure or practice: Texas BHEC Contact Page
- File a complaint with the Texas Attorney General’s Consumer Protection Division: File a Complaint
California:
Consumers have the right to:
- Request health care records by submitting a written ROI including identifying information and a description of the records requested.
- Contact the California Department of Public Health (CDPH) to file a complaint about a health facility: CDPH File a Complaint
- Contact the appropriate licensing board via the California Department of Consumer Affairs for professional complaints: California DCA Boards
